What Is a Comprehensive Health Assessment?
Pathway to Care and Recovery Licensed Practical Nurses (LPN) are on staff to assist in monitoring your vitals, assess for signs of withdrawal symptoms or complications, medication side effects, medication issues, and emerging signs of medical or mental health concerns.
Our nursing staff will provide a one-to-one assessment and supply health and wellness education. They may make referrals and help schedule medical appointments with your primary care physician, specialist, or link you to an healthcare facility by making direct contact with the healthcare staff. Additionally, if your physical health condition presented warrants emergency medical intervention, the staff will contact emergency services.
How Health Assessments Support Addiction Recovery
A Comprehensive Health Assessment is an important piece of your Level of Care Assessment (LOCA). A Pathway to Care and Recovery LPN completes interviews and physical assessments of all individuals seeking services. The LPN collects information that is included in Dimension 2: Biomedical Conditions and Complications of the LOCA. The medical information collected is necessary to finding the appropriate treatment facility for you.
A holistic health evaluation assesses your physical, mental, emotional, and spiritual health. It also considers how these aspects are connected to substance use.
The Key Components of a Holistic Health Evaluation
Our LPNs are able to interact effectively with you at all stages of substance use/recovery. They are able to effectively and safely work with individuals who are actively intoxicated, are in varying stages of detoxification, or impairment/withdrawal from drug use. Our LPNs are able to help you by interpreting medical information, assisting with prescription needs, and with medication-assisted treatment (MAT) protocols and the maintenance of these protocols.
Your health and safety is our priority. LPNs monitor all individuals at Pathway to Care and Recovery for signs of withdrawal symptoms/complications, medication side effects/medication issues, and emerging signs of medical issues/ mental illness.
